Job Summary:
Responsible for the planning, direction, coordination, general supervision, budgeting, and management of all assigned residences within the Memory Care Community. Identifies and interprets resident needs and manages the services to achieve community and regulatory goals and standards.
Essential Duties:
- In compliance with the Company's policies, is responsible for interviewing, hiring, developing, training, supervising, scheduling, and following the Company's performance management policy with assigned staff.
- Establishes and ensures that staffing levels and scheduling patterns are adequate and have appropriate coverage at Community.
- Investigates and documents incidents, identifies safety hazards and institutes corrective action in a timely manner.
- Investigates and ensures completion of internal and state required incident reports including completion of appropriate interventions in a timely manner.
- Participates in the Assisted Living's Quality Assessment and Assurance Program. Initiates risk management action plans to ensure quality care goals are met.
- Performs pre-admission assessments of all potential residents. Assists in orienting new residents to the physical environment and community culture.
- Coordinates move-in, transfer, and discharge of Memory Care residents.
- Initiates and coordinates discharge planning process for residents of Memory Care.
- Ensures availability of adequate and appropriate supplies and equipment. Develops and manages the departmental budget process and supervises revenue and expenses regularly to remain within established budgetary guidelines.
- Monitors the health needs of residents providing appropriate and timely intervention.
- Reports significant resident changes to appropriate licensed individuals. Maintains current medical records and ensures they comply with regulatory guidelines.
- Takes a proactive role to promote person centered care and culture change initiatives.
- Develops and implements departmental policies and procedures. Interprets departmental policies to all appropriate individuals.
- Schedules and chairs care conferences with all residents and families, and other appropriate
- staff. Monitors resident services and assesses effectiveness of care programs. 1
- Completes non-clinical assessments per State and company policies. ii
- Assists in the development of service plans that are person centered. ti
- Plans and schedules State required staff training programs for Memory Care staff.
- Attends all required training, in-service, and staff/community meetings.
- Maintains a positive and professional demeanor toward residents, visitors, families, and co workers.
- Other duties as assigned.
Basic Qualifications & Experience:
- One year experience in Memory Care setting.
- 3 years' experience in Senior Care or an Assisted Living setting.
- 1-year experience in a supervisory capacity.
- Must be knowledgeable of Colorado state assisted living regulations.
- Must have computer processing and electronic medical database experience such as Point Click Care.
- Must have excellent written and verbal communication skills.
- Must be able to read, write and speak the English language.
- Must possess excellent customer service skills.
Working Conditions/Physical Requirements
- Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
- Possible exposure to chemicals, blood borne pathogens and other communicable diseases.
- Sits, stands, bends, and moves frequently during working hours.
- Is subject to frequent interruptions.
- Interacts with residents, family members, visitors frequently.
